Mục tiêu: Đánh giá giá trị của 3 thang điểm CATCH, CHALICE, PECARN. Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang, hồi cứu và tiến cứu trên 167 bệnh nhân (BN) từ 1 tháng tuổi - 16 tuổi bị chấn thương sọ não (CTSN) tại Khoa Cấp cứu, Bệnh viện Nhi Trung ương từ 01/2017 - 4/2018. Kết quả: Tỷ lệ chụp cắt lớp vi tính (CLVT) với CHALICE là 89,2%, không có sự khác biệt rõ ràng về tỷ lệ có tổn thương trên CLVT giữa nhóm nguy cơ cao và nguy cơ thấp trong CHALICE. Tỷ lệ chụp CLVT với CATCH là 86,2%, nhóm nguy cơ cao của CATCH có tổn thương trên CLVT cao gấp 2,35 lần so với nhóm nguy cơ trung bình. Tỷ lệ chụp CLVT với PECARN là 85,6%. PECARN nguy cơ cao có tỷ lệ tổn thương trên phim CLVT ở nhóm < 2 tuổi (79,2%) và ≥ 2 tuổi (73,0%) cao hơn so với PECARN nguy cơ trung bình ở cả 2 nhóm (50,0% và 46,9%). Thang điểm PECARN có độ nhạy cao nhất: Ở nhóm < 2 tuổi và ≥ 2 tuổi lần lượt là 97% và 95%. Độ nhạy của CATCH là 91,0%, CHALICE là 90,0%. Kết luận: Thang điểm PECARN dễ sử dụng, sát với thực tế lâm sàng, xác định trẻ CTSN ngay cả khi có nguy cơ thấp.

Abstract
Objectives: To evaluate the values of CATCH, CHALICE and PECARN scale. Subjects and methods: Retrospective cross-sectional descriptive and prospective study on 167 patients aged from 1 month to 16 years old with traumatic brain injury at the Emergency Department, Vietnam National Children’s Hospital from January, 2017 to April, 2018. Results: The rate of patients performing CT-scan with CHALICE was 89.2%, there was no significant difference in the rate of lesions on CT-scan between high-risk group and low-risk group in CHALICE. The rate of patients performing CT-scan with CATCH was 86.2%, the rate of lesions on CT-scan in the high-risk group was 2.35 times higher compared to the moderate risk group. The rate of patients performing CT-scan with PECARN was 85.6%. The high-risk group of PECARN in under 2-year-old children (79.2%) and over 2 year-old-and-older children (73.0%) had higher rate of lesions on CT-scan compared with the moderate risk group (50.0% and 46.9%, respectively). The PECARN score was the most sensitive (97% in the group of less than 2 years and 95% in the group of 2 years and older). The sensitivity of CATCH was 91.0%, CHALICE was 90.0%. Conclusion: PECARN scale is easy to use and convenient in clinical practice which helps identify trauma brain injury in children with low risk.
